A 32-year-old woman, gravidal para 0, was referred to our hospital at 35 weeks' gestation with a chief complaint of severe epigastric pain. Serum and urine amylase levels were 360 and 4800 IU/l, respectively, and the serum triglyceride level was 7533 mg/dl. Therefore, she was diagnosed as having acute pancreatitis in pregnancy with hyperlipidemia. Cesarean section and abdominal drainage were performed. Postoperative recovery was uneventful, and the patient was well after surgery. Only 78 cases of acute pancreatitis in pregnancy have been reported in Japan, with multiparas more common than primiparas. Of these, 65.7% was presented in the third trimester. Neonatalmortality was 32.8%, and maternal mortality was 11.9%. In cases of acute pancreatitis in pregnancy, especially with hyperlipidemia, the prognosis is poor, surgical treatment such as cesarean section is recommended above conservative therapy.
