Surgical Treatment for Early Colorectal Cancer and Indications for Laparoscopic Assisted Colon Resection

Recent advances in polypectomy for early colorectal cancer have decreased the number of surgical operations. Minimally invasive surgery is needed for these patients to preserve rectal function and improve the postoperative quality of life. From 1980 to 1992, 622 colorectal cancer patients underwent surgery in our department. Sixty-two patients (10%) had early stage cancer. Cancer tissue invaded to the mucosal layer in 26 patients, and to the submucosal layer in 36. Two patients in whom cancer tissue invaded to the submucosal layer, had paracolic lymph node metastasis. Recently, trans-sacral resection and trans-anal resection have been performed for early rectal cancer. On the other hand, laparoscopic assisted colon resection has been performed for early colon cancer. This procedure was less invasive and was able to dissect the paracolic lymph node with ease. Laparoscopic assisted colectomy was useful and highly acceptable for early colon cancer.
