A case of pseudomixoma peritonei of the ileum is reported. A 60-year-old man was admitted to our hospital complaining of abdominal pain and fever. The diagnosis of peritonitis with an inflammatory reaction was made. Emergency laparotomy revealed a jelly-like material with an abscess formation at the mesenteriy of the ascending colon, and a right hemicolectomy was performed. Although histopathologically mucin-producing well differentiated adenocarcinoma was partly detected, a diagnosis of pseudomyxoma peritonei was made without determining its origin. Five months after the operation, we noticed a nodular mass which was diagnosed as a recurrence of the pseudomyxoma peritonei. The second operation revealed a solid tumor at the ileum, so the ileum including the tumor was resected. Histopathologically, the tumor of the ileum was confirmed to be the origin in this case. In conclusion, in a case where peudomyxoma peritonei is evident, careful examination of the small intestine should be carried out to determine its origin.
