Laparoscoy-Assisted Ileal-Pouch Anal-Canal Anastomosis for Ulcerative Colitis

We treated 776 patients with ulcerative colitis, 119 of whom required surgery. Ten patients underwent laparoscopy-assisted ileal-pouch anal-canal anastomosis (LAIACA) and 18 conventional open ileal-pouch anal-canal anastomosis (OPIACA). We compared clinical factors between the 2 procedures. Operating time was 377 minutes in LAIACA and 308 minutes in OPIACA, while intraoperative blood loss was 177 ml in LAIACA and 363 ml in OPIACA. Eating was started 10.3 days after LAIACA surgery and 19.2 days after OPIACA surgery. The most significant difference was noted in duration of hospitalization from start of eating to discharge, 14.9 days in LAIACA and 23.8 days in OPIACA. LAIACA provides more rapid recovery, less invasiveness and a shorter hospital stay than OPIACA.
