A Case of Internal Supravesical Hernia

A 66-year-old man with no history of abdominal surgery admitted for lower abdominal pain and vomiting was found in abdominal computed tomography (CT) to have the dilated small intestine pressing the bladder with caliber change in the small intestine. Internal hernia was suspected and emergency surgery was done, establishing a diagnosis of strangulated ileus due to incarceration of the ileum in the internal supravesical hernia. Due to the patient's poor condition, we conducted partial resection of the ischemic ileum, removal of the hernia sac, closure of the hernia orifice, and ileostomy. His postoperative course was uneventful. He underwent surgery to close the ileostomy on hospital day 16 and was discharged on hospital day 24. Internal supravesical hernia is very rare, with our case being only the 13th reported in the Japanese literature, and no case in which final diagnosis was achieved preoperatively. Characteristic CT findings were useful in preoperative diagnosis.
