We report a case of primary T-cell malignant lymphoma of the ileum diagnosed preoperatively. A 64-year-old man hospitalized for right lower quadrant pain and constipation demonstrated ulceration of the ileum on colonoscopy. Immunohistochemical staining of biopsy specimens showed the presence of T-cell malignant lymphoma. Radiographic studies of the small intestine elucidated fistula formation between the ileum and ascending colon. The huge ulceration and coloenteric fistula of the ileocecal region necessitated right hemicolectomy. T-cell malignant lymphoma is rare, and its prognosis is much poorer than that of B-cell lymphoma. We performed 4 courses of CHOP as adjuvant chemotherapy. The patients remains alive 2 years and 8 months postoperatively.
