We report a case of gastrointestinal hemorrhage due to localized hyperplastic gastritis associated with giant gastric folds. A 44-year-old woman admitted for malaise and tarry stools was found in upper gastrointestinal endoscopy to have a hemorrhagic ulcer at the upper stomach, which was treated hemostatically. The next day, the ulcerated lesion showed a semispherical appearance with enlarged gastric folds. We suspected a submucosal tumor of the stomach, specifically gastrointestinal stromal tumor (GIST), but histological examination of biopsy specimens, CT, MRI, and endoscopic ultrasonography (EUS) did not demonstrate concomitant findings. Further endoscopic examination showed a nodular or oblate lesion rimmed by a precipitous verge at the lesser curvature of the upper stomach, which was mistaken for malignant lymphoma, resulting in total gastrectomy. Histologically, the lesion was diagnosed as localized hyperplastic gastritis associated with foci of heterotopic gastric glands.
