We report a very rare case of tailgut cyst associated with malignant findings. A 33-year-old woman was seen for metrorrhagia, in which a cystic pelvic tumor was found in ultrasonography and MRI. The tumor, located in the presacral space, adhered firmly to the coccyx. We resected the cystic tumor through a transabdominal approach. The pathological diagnosis was a tailgut cyst with mucinous carcinoma including epithelial cells.
