A Case of Transmesocolonic Hernia in Mesentery of Transverse Colon

We present a rare case of transmesocolonic hernia in the mesentery of transverse colon. A 74-year-old man undergoing hemodialysis and with no history of surgery was referred for abdominal pain and vomiting. In abdominal computed tomography and gastrographin contrast examination using a long tube, he was found to have complete stricture due to a transmesocolonic hernia in the mesentery of transverse colon. Laparotomy showed an oval hernial orifice about 3 cm in diameter in the mesentery of the right transverse colon, with about 10 cm of the small intestine invaginated through the opening into the mesentery. The invaginated intestine was reverted by manupulation and had no necrosis, so the orifice was repaired by suture. Transmesocolonic hernia in the mesentery of transverse colon is rare, and should be considered a differential diagnosis of ileus under abdominal computed tomography and gastrographin contrast examination.
