Pericardial tamponade due to pericardial metastasis from rectal cancer is extremely rare. We report a case of pericardial tamponade caused by such metastasis. A 54-year-old man who had undergone Miles operation for rectal cancer about one year earlier, and admitted to the hospital for severe cough and dyspnea, was found in computed tomography (CT) of the chest to have effusion of the pericardial space diagnosed with cardiac tamponade. Pericardiocentesis was carried out, and the cytology of the effusion showed class V. He was treated with intrapericardial chemotherapy using adriamycin and OK-432, and has not suffered from pericardial effusion since chemotherapy. Intrachemocardial chemotherapy using adriamycin and OK-432 is thus useful in treating malignant pericardial effusion.
