A Case of Small Liver Cancer Combined with Early Cancer of the Stomach

A case of resectable synchronous double cancer of the liver and stomach is reported. A 56-year-old man was admitted to our hopsital with a complaint of epigastralgia. Endoscopic examination showed an irregular depression at the lesser curvature of the antrum. Furthermore, ultrasonography, CT scanning and angiography demonstrated a solid mass about 2×2 cm in diameter in the posterior segment of the liver. Under the diagnosis of double cancer, subtotal gastrectomy and partial resection of the liver was performed on November 24, 1988. The resected specimen disclosed a type IIc early cancer of the stomach measuring 2.5×1.5 cm and an 1.7×1.2×1.0 cm encapsulated tumor of the liver. Histological examination revealed tubular adenocarcinoma limited to the mucosal membrane of the stomach and hepatocellular carcinoma associated with liver cirrhosis. While the postoperative course was uneventful, recurrence of hepatocellular carcinoma was revealed by abdominal CT scanning a year after the operation. The patient has been treated by hepatic arterial chemotherapy using an implanted cannula and has been well for over 16 months after the operation. Twenty-seven cases of synchronous double cancer of the liver and stomach including our case have been reported in the Japanese literature. To our knowledge, this is the third case of small liver cancer combined with early cancer of the stomach in Japan.
