A case of primary gastric actinomycosis in which the preoperative diagnosis was difficult is reported. A 62-year-old woman who complained of nausea, epigastric pain, and weight loss was admitted to our hospital. Abdominal ultra-sonography, computed tomography, MRI, and gastroduodenoscopy revealed a primary gastric tumor that was suspected of being malignant. Distal gastrectomy with transverse colectomy was performed. Histopathological studies of the resected material revealed characteristic sulfur granules in the indurated mass, and a diagnosis of gastric actinomycosis was made. No evidence of malignancy was found. Piostoperative antibiotic therapy was performed for only 1 week, and there has been no evidence of recurrence. In recent years, this disease are relatively rare, and it is not always easy to diagnose this disease. But it is important to be taken into consideration in the differential diagnosis of the abdominal mass.
