A Case of Primary Malignant Lymphoma of the Spleen Associated with Hepatocellular Carcinoma

A 49-year-old man who had been diagnosed as having liver cirrhosis and splenomegaly was admitted to Chiba Rosai Hospital with the complaint of left lateral chest pain. Malignant lymphoma of the spleen was suggested by abdominal echography, computed tomography and 67Ga-scintigraphy. There were three tumor nodules in the spleen which were solid, elastic-hard, and well-defined, and measured 3-7 cm in diameter. Lymphadenopathy was seen only in the lymph node of the splenic hilus. The histological diangosis was malignant lymphoma, diffuse large cell type (B cell). Six courses of postoperative chemotherapy were given out. Two years after splenectomy hepatocellular carcinoma was found and percutaneous ethanol infusion therapy was performed. There is no evidence of recurrence of malignant lymphoma, and the hepatocellular carcinoma has also been well controlled during the 4 years since the splenectomy. This is the first report of the primary malignant lymphoma of the spleen associated with hepatocellular carcinoma in Japan.
