Perforation of Meckel's diverticulum is very unusual, and may be caused by heterotopic tissue, a foreign body, a tumor or Crohn's disease. Perforation of Meckel's diverticulum by a fish bone is by far the rarest occurrence: A review of the Japanese literature revealed that only 4 cases have been reported to date. The following case report of a 4-year-old boy brings the total to 5. In the present case, it is of particular interest that both ends of the fish broke through the wall of Meckel's diverticulum. The diverticulum was excised, and histological examination revealed no heterotopic tissue.
