A Case of Common Bile Duct Stricture due to Blunt Abdominal Trauma

Common bile duct stricture due to blunt abdominal trauma is rare. We experienced such a case in a 25-year-old woman. On May 6, 1991, she was injured in a traffic accident and was admitted to our hospital with liver and pancreas trouble. After 6 days she was discharged. But 17 days after the accident, she was readmitted complaining of epigastralgia and jaundice. Laboratory examinations revealed a T-Bil, level of 4.0 mg/dl and moderate liver dysfunction. Abdominal US and CT examination showed marked dilatation of the gallbladder and common bile duct, and swelling of the head of the pancreas, but no tumor was found, PTCD was performed. Tube cholangiography and ERCP revealed severe stenosis at the middle of the common bile duct, but no abnormalities were seen in the pancreatic duct. Bile cytology showed no malignancy. She was diagnosed as having a benign stricture of the common bile duct due to the traffic accident. PTCD was performed for about 1 month but the stenosis was not improved. Fifty-eight days after the injury, she underwent cholecystectomy and choledochoduodenostomy. She is now very well, about 1 year after the operation.
