A Case of Hemosuccus Pancreaticus due to Chronic Pancreatitis Diagnosed by Color Doppler Echography

We recently experienced a case of hemosuccus pancreaticus due to chronic pancreatitis diagnosed by color Doppler echography. A 46-year-old man with chronic pancreatitis was admitted to a hospital because of shock. The chief complaints were left hypochondralgia and melena. Ultrasonography showed calcification of the pancreas, dilatation of the pancreatic duct and a 26×28 mm cyst in the pancreas tail. Color Doppler echography showed abnormal flow of the cyst and it was diagnosed as an aneurysm. Selective splenic arteriography revealed a splenic aneurysm. Therefore hemosuccus pancreaticus due to a splenic pseudoaneurysm was suspected, and distal pancreatectomy and splenectomy were performed. Histological examination revealed a pseudocyst with a thrombus and a dilated main pancreatic duct with a thrombus and the diagnosis was hemosuccus pancreaticus due to a pseudoaneurysm.
