A Case of Markedly Elevated Serum CA19-9 with Choledocholithiasis

A 7l-year-old female presented with vomiting. Laboratory tests on admission revealed leukocytosis, jaundice, and marked elevation of serum CA19-9 (89,305 U/ml). An abdominal ultrasonogram, CT scan and PTC showed bile duct stones. No tumors were suspected. Serum CA19-9 gradually decreased to 145 U/ml after biliary drainage. The patient was operated on for bile duct stones. The gallbladder exhibited wall thickening and had mixed stones in it. Pathology revealed no evidence of malignancy. Immunohistological staining for CA19-9 demonstrated it to be positive in the gallbladder wall.
