A Case of Zenker's Diverticulum Associated with Thyroid Adenoma

Zenker's diverticulum is occasionally encountered but rarely operatd on. We report a surgical case of Zenker's diverticulum associated with a thyroid adenoma. The patient is a 77-year-old man with a complaint of dysphagia for five years. he has been aware of a cervical tumor for one year. Cervical computed tomography showed a thyroid tumor of the right lobe and an oval gas collection on the left of the trachea which was characteristic of Zenker's diverticulum. Zenker's diverticulum was confirmed by esophagography and esophagoscopy. Diverticulectomy and right thyroid lobectomy were performed. The thyroid tumor was 90×70×35 mm in size and was histologically proved to be follicular adenoma. The resected diverticulum was 25×15 mm in size and was composed of squamous epithelium and a thin muscular layer. Zenker's diverticulum is regarded as a pulsion diverticulum. In our case, it is suggested that the incidentally associated thyroid tumor enlarged the Zenker's diverticulum, causing dysphagia by compressing the trachea and esophagus.
