A Case Report of Human Fascioliasis Revealed as Retroperitoneal Lymphadenitis

We experinced a case of human fascioliasis with retroperitoneal lymphadenitis indistinguishable from pancreas head cystic tumor. A 49-year-old woman was admitted with high fever. The laboratory data revealed eosinophilia and an elevated level γ-glutamyltranspeptidase (GTP). Computed tomography (CT) showed a cystic lesion about 4 cm in diameter on the pancreas head. We suspected a pancreas head tumor and an operation was performed. The tumor separated from the pancreas head was resected with the paraaortic lymph nodes. Histopathologically the tumor was suppurative lymphadenitis with infilted eosinophils. Two months after the operation, a cluster of multiple liver abscesses was detected by CT. The diagnosis of fascioliasis was established a serological method (Ouchterlony test). After treatment with Bithionol, the multiple liver abscesses and retroperitoneal lymphadenitis disappeared.
