Inflammatory pseudotumor (IPT) of the esophagus is a very rare disease. A case of IPT of the esophagus with progression of stenosis is reported. A 63-year-old man was referred with hematemesis. Upper gastrointestinal radiography revealed stenosis of the distal esophagus in another hospital. The patient was admitted to our hospital for treatment of an esophageal tumor. Endoscopic study demonstrated multiple esophageal ulcer scars and obstruction at 35 cm from the incisors. Enhanced computed tomography confirmed wall thickening. Esophageal resection was required because of stricture development in the distal esophagus. The resected specimen revealed ulceration and wall thickening. Histopathological examination showed massive infiltration of lymphocytes, histiocytes, plasma cells and eosinophils accompanied by reactive blood vessels and fibroblasts. These findings confirmed the diagnosis of IPT.
