A Case of Intussusception due to an Inflammatory Fibroid Polyp in the Ileum

We report a case of inflammatory fibroid polyp (IFP) of the ileum with intussusception. An 80-year-old woman was admitted to the hospital for abdominal pain and abdominal fullness with a provisional diagnosis of intestinal obstruction. Contrax X-ray study through a long intestinal decompression tube and abdominal CT scanning indicated intussusception of the ileum. On laparotomy, ileo-ileal intussusception was found. The leading segment of the intussusception consisted of a hard rubber-like mass, about 60 cm proximal to the ileocecal valve. Partial resection of the ileum including the mass was performed. The mass was pedunculated and measured 54×48×36 mm, with ulceration of the covering mucosa at the top. The mass was confined mainly to the submucosal layer. a diagnosis of inflammatory fibroid polyp was made based on the histological features consisting of proliferating fibrous connective tissue and small bllod vessels, with inflammatory cell infiltration. IFP of the ileum is rare and frequently causes intussusception (in 84.8% of the reported cases). The 82 cases reported in Japan, including our case, are reviewed.
