A 50-year-old woman who occasionally experimenced abdominal pain and distention at the time of menstruation. She was ultimately referred to our hospital in February, 2000. After decompression of the intestinal tract with a Denis tube, enterography revealed a stenotic shadow in the terminal ileum, and laparotomy was performed for a preoperation diagnosis of ileus after appendectomy. At operation, stenosis of the ileum 10cm proximal to the ileocecal junction and a myoma and chocolate cyst of the right ovary were identified. Partial ilectomy and total hysterectomy and bilateral salpingooophorectomy were performed. Histopathological examination showed endometriosis affecting the right ovary, uterus, and the ileum, with marked fibrosis of the ileum causing obstruction. We reported the ileus caused by endometriosis of the ileum and review the 35 cases in the Japanese literature.
