The Problem of Autologous Blood Transfusion for the Patients who Underwent Preoperative Adjuvant Chemotherapy: 5-FU and CDDP Concentration in the Preserved Autologous Blood

To confirm the safety of autologous blood transfusion in patients of advanced esophageal and gastric cancer undergoing preoperative adjuvant chemotherapy (5-FU: 500 mg/m2/day, continuous infusion on days 1-5 and days 8-12, CDDP: 5 mg/m2/day, drip infusion on days 1-5 and days 8-12), we analyzed 5-FU and CDDP concentration in preserved autologous blood and evaluated toxicity with changes in potassium concentration. All data on the 5-FU concentration was below the limit of measurement (<1 ng/ml). The CDDP concentration showed the effective dose from 0.55±0.25 μg/ml to 0.57±0.25 μg/ml. Changes in potassium concentration in preserved autologous blood and in non irradiated homologous blood were similar. In this study of a low-dose CDDP regimen, the CDDP concentration in preserved autologous blood apparently had no adverse effect on patients. In treatment with a high-dose CDDP regimen, however, it is important to collect autologous blood after the CDDP concentration has decreased sufficiently.
