Risk Factors of Postopertative Complications and Prognostic Factors in Patients with Colorectal Cancer over 85 Years Old by Multivariate Analysis

Surgery on patients in their 70s for colorectal cancer is common, but comparatively rare in those 85 years old. We compared clinicopathological factors, postoperative complications and survival in 22 patients over 85 years old and 103 75-84 years old to clarify issues that may require attention in such older patients. We also examined risk factors in postoperative complications and prognostic factors using multivariate analysis. Preoperative ileus/perforation and postoperative respiratory complications, cerebromental complications and death from other disease were more frequent and lymph node dissection limited in those over 85 years old, but no significant difference was seen in survival rate, mortality, or morbidity. Multivariate analysis indicated that age exceeding 85 years was a risk factor for postoperative cerebromental complications. Location and histological type of colorectal cancer, liver metastasis, peritoneal dissemination and lymph node metastasis were prognostic factor, but age was not. Because no difference was seen in survival, mortality, or morbidity between the 2 groups, patients over 85 years old can be treated similarly to those 75-85 years old. Age exceeding 85 years is, however, a risk factor in postoperative cerebromental complications and age increases death from other disease. Attention to these factors in patients older than 85 is thus merited in postoperative care and follow-up.
