We experienced 2 cases of small signet-ring cell carcinoma that had developed in the gastric remnant (GR) gastrectomized after early gastric cancer. Case 1 was a 51-year-old woman initially undergoing surgery 1.5 years earlier. Case 2 was a 49-year-old man first operated on 7.5 years before. Both had been absolute curative resections. The 2 lesions in the GR were treated with endoscopic mucosal resection (EMR) and showed a similar endoscopic appearance as a small flat localized white spot on the mucosal surface of the nonstump area of the GR. These 2 cases underscore the importance of endoscopic follow-up and the indication of EMR for treating carcinoma of the GR.
