A Case Report of Adult Hepatoblastoma with Preoperative Radiotherapy

We present a very rare case of adult hepatoblastoma with preoperative radiotherapy. A 60-year-old man came to our hospital because of liver dysfunction. On admission, ultrasonography showed a mass lesion, 15 cm in diameter, in the right lobe of the liver. The alpha-fetoprotein (AFP) level was 11,932ng/ml, and we made a diagnosis of hepatocellular carcinoma. Radiotherapy with a dose of 40 Gy was performed, and the tumor shrank to 6 cm, in diameter, and the AFP level decreased to 44 ng/ml. The AFP level later increased again and we performed right hepatic lobectomy with partial resection of the diaphragm one year after the initial radiotherapy. The tumor was the nodular type, measured 11×7. 5×6 cm, and was hemorrhagic and necrotic. Histologically, most of the tumor was composed of embryonal and anaplastic hepatoblastoma, but hepatocellular carcinoma was detected in a small portion of the tumor. The patient left the hospital on post operative day 22, but died of multiple metastasis to the left neck and both lungs 2 months after the operation.
