Three Cases of Hepatocellular Carcinoma with Intraductal Growth

We encountered three resected cases of hapatocellular carcinoma (HCC) with intraductal growth to the extrahepatic bile duct. All three cases were male with HB virus and one case was cirrhosis. In all cases, the intraductal tumors was visualized in the extrahepatic bile duct by cholangiography. Radical repatectomy with removal of the intraductal tumor was carried out. The intraductal tumor could be removed after choledochotomy as it had no tendency toward infiltrative growth into the ductal wall. All three patients remain well without any evidence of recurrence 5 months, 6 months, and 12 months later, respectively. HCC is often associated with intraductal growth in its advanced stage and its prognosis is poor. But it is important that adequate hepatic resection with removal of the intraductal tumor be carried out in resectable cases, following a precise preoperative diagnosis.
