A 72-year-old man was found by computed tomogram (CT) to have a cystic liver lesion 3 cm in diameter. Magnetic resonance imaging (MRI) showed a tumor in the cystic liver lesion, not connected with the intrahepatic bile duct. Intraoperative ultrasonography (US) showed a papillary tumor arising from the intrahepatic bile duct wall with topically cystic dilation. We conducted partial hepatectomy. Histological findings showed tubulopapillary adenoma of the topically and cystically dilated liver bile duct. We should consider such disease in diagnosis of the cystic liver disease.
