A Case of Rectal and Ureter Stricture due to Pelvic Peritonitis

A 57-year-old woman admitted for abdominal distension and constipation was found in double-contrast studies to have complete obstruction in a long Ra lesion of the rectum. Colonoscopic study demonstrated complete stricture of the rectum about 15 cm from the dentate line, but rectal mucosa were smooth. Computed tomography (CT) confirmed rectal wall thickening with an unclear tumor in front of the sacrum. Hydronephrosis of the right kidney with stricture of the ureter on right side was found. Tumor markers were within normal range. Surgery conducted for suspected diffuse infiltrated rectal cancer showed that the main tumor was located in the left ovary and invaded the rectum, ileum, and right ureter. Low anterior resection with partial resection of the ileum and the right ureter were done. Histopathological findings showed no malignancy, but found endometriosis. Inflammatory granulation in the centralis of the ovary with actinomycosis caused stricture of the rectum. The postoperative course was good.
