We report a case of the esophagus cancer with the right aortic arch. A 58-year-old man found to have an abnormality in esophagoscopy was found in radiography and computed tomography (CT) to have an abnormal aortic arch. Esophagoscopy showed early esophageal cancer, necessitating left thoracotomy. The cancer was IIIb Stewart's type. It occurred due to pressure by the left ductus arteriosus. A review of the literature showed that only 7 examples including our case, had been reported of esophagus cancer in the right aortic arch.
