A Case of Duodenal Lipoma with Laparoscopy-assisted Tumor Resection -Analysis of Reported 109 Cases of Duodenal Lipoma in Japan-

A 53-year-old woman was found in endoscopy to have a bulgy, yellowish tumor with a smooth surface at the second portion of the duodenum was diagnosed with duodenal lipoma. The tomor was 30 mm across and had a wide 20 mm peduncle. Given the tumor size and wide peduncle, endoscopic polypectomy was not attempted due to the risk of bleeding and perforation. The tumor was resected with laparoscopic assistance. The first step in this operation is detaching the duodenum up to the front of the descending aorta, and lifting it out of abdomen through the extended paramedial wound. The second step is tumor resection via a small incision in the duodenal wall. The pathological diagnosis was lipoma. This procedure is very useful in treating benign duodenal tumors, which exists at the second portion or entrance of the third portion of the duodenum. It may feasibly be applied to local resection of carcinoma of the duodenal papilla.
