We report a rare case of strangulated ileus caused by appendix epiploicas. A 61-year-old man admitted for sudden abdominal pain diagnosed as ileus had undergone cholecystectomy in 2001. Clinical findings and abdominal ultrasonography suggested strangulated ileus, necessitating emergency surgery. We found the small intestine to be strangulated about 80 cm from the terminal ileum due to a band formed from appendix epiploicas of the sigmoid colon. The serosa of the sigmoid was torn. Strangulation was relieved, enabling us to avoid resecting the damaged small intestine.
