We present a rare case of advanced jejunal cancer with good response to TS-1 chemotherapy. A65-year-old woman admitted for appetite loss and vomiting, was found in enhanced abdominal CT to have thickening of jejunum wall and a large tumor mass on the right side of the ascending colon. Contrast examination of the small intestine showed local jejunum stenosis and dilation at its oral site. Chest CT showed two metastatic tumor nodules at right S8a 12 mm in diameter and the left S1+2 5 mm in diameter. Partial jejunectomy to reduce her ileus syndrome showed that the 5×3 cm jejunal tumor measuring was mucinous adenocarcinoma with multiple disseminated nodules in the peritoneum. Postoperative chemotherapy with TS-1 for 24 months gradually reduced X-ray shadows of peritoneal and pulmonary metastases and she remains apparently healthy at present.
