Polycythemia vera (PV) is an idiopathic malignancy involving an increase in hematopoietic stem cells. PV is thought to be equivalent to chronic low-grade disseminated intravascular coagulation (DIC). Surgical results invariably involve secondary complications such as thrombosis and bleeding. A 61-year-old woman diagnosed with PV 11 years earlier underwent hemodialysis and steroid treatment for renal failure. She was diagnosed with panperitonitis due to colon perforation and underwent emergency surgery that revealed stercoral perforation of the rectosigmoid colon. After strict postoperative management, she was discharged on postoperative 40 day.
