We report a case of strangulated small bowel obstruction caused by adhesive residual appendix after laparoscopic appendectomy of the ileum. A 45-year-old woman undergoing laparoscopic appendectomy elsewhere was seen for abdominal pain, diagnosed as strangulated small bowel obstruction. In surgery, we found the residual appendix had adhered to the ileum, strangulating it. We resected the adhesion and the ischemic ileum and cecum including the residual appendix. In laparoscopic appendectomy, the stump had not inverted into the cecum, and this possibility should be considered in cases of a residual appendix.
