Obturator hernia is an uncommon but important cause of intestinal obstruction. Adult malrotation is difficult to diagnose because of its rarity. We report a 76-year-old woman with abdominal pain and vomiting and constipation whose malrotation was confirmed in emergency long tube insertion. Computed tomography showed a right obturator hernia. Abdominal exploration showed a great portion of small intestine involving. Richter's small intestine incarcerated within the right obturator hernia. No evidence of bowel perforation was noted. Reduction of the volvulus and freedom from Richter's hernia resulted in the return of normal color to the affected segments of intestine, and we conducted on the appendectomy.
